In collaboration with Nurse.com, AAACN is pleased to offer a new option to nurses preparing for the Medical-Surgical Nursing Certification Board (MSNCB) Certified in Care Coordination and Transition Management (CCCTM) Exam.

This program includes 13 individual online modules from the AAACN CCTM course (total of 26.4 contact hours), 3 live webinars, including sample questions (total of 3 contact hours) and a practice test with 65 additional sample questions (total of 1.1 contact hours). TOTAL CONTACT HOURS = 30.5.

Nurses are integral and influential for helping patients navigate within and between healthcare systems. There is a significant need for nurses who can concentrate on care coordination and transition management (CCTM) activities due to the risk of negative outcomes (due to incomplete information exchange, misunderstanding, and missing key factors of strategic plans of care).

Coordinating patient care and managing patient transitions are part of different job responsibilities within nursing (e.g., navigators, care managers, discharge planners, or coordinators). All nurses have some connection to coordinating care or managing transitions of care for patients.

Course Agenda

The content will help you prepare for the certification exam and will be available until the series ends on January 11, 2018 at 11:59 PM ET.

  • Online Modules**: All online modules are available on November 6, 2017. Each module includes a PDF version of the corresponding text chapter of the AAACN Care Coordination and Transition Management Core Curriculum.

    • Education and Engagement of Patients and Families – 2.0 contact hours
    • Coaching and Counseling of Patients and Families – 1.8 contact hours
    • Patient-Centered Care Planning – 1.9 contact hours
    • Support for Self-Management – 1.8 contact hours
    • Nursing Process: Proxy for Monitoring and Evaluation – 1.6 contact hours
    • Teamwork and Collaboration – 1.9 contact hours
    • Cross-Setting Communications and Care Transitions – 1.7 contact hours
    • Population Health Management – 2.0 contact hours
    • CCTM Between Acute Care and Ambulatory Care – 2.7 contact hours
    • Informatics Nursing Practice – 2.2 contact hours
    • Telehealth Nursing Practice – 2.4 contact hours

**Certificates and contact hours for the online modules will be provided by AAACN for each individual module.

  • Practice Test***: Available starting November 6, 2017.
  • Interactive Classroom: Weekly discussion questions are provided. You will also be able to network with peers and speakers, available to you throughout the series.

Speakers

Traci Haynes, MSN, RN, BA, CEN, CCCTM, has been a nurse for over 30 years. Her nursing experience includes emergency nursing in a level 1 trauma center, labor and delivery nursing, clinical oversight for wound healing centers, and telehealth nursing. She recently served as the project manager and co-editor for the Care Coordination and Transition Management (CCTM) Core Curriculum of the American Academy of Ambulatory Care Nursing (AAACN).

Amy Lipsett, BSN, MHA, RN, ONC, CCCTM, is a Staff Development Nurse at Thomas Jefferson University Hospital. In her role, she guides staff nurses in providing optimal care to a population that includes orthopedic, trauma, and medical-surgical patients. Amy is responsible for developing and maintaining both staff and patient education. She enjoys writing nursing lessons and dabbling in legal nurse consulting.

Laura Tesch, MSN, BA, RN, CCCTM, is a Community-Based Case Manager for Aurora Health Care in Wisconsin. Previously, she was a clinical nurse specialist clinical instructor and preceptor. Laura was nominated as a teacher of the year at the University of Wisconsin, Milwaukee. She received awards for outstanding service and personalized care from St. Luke's Medical Center and celebration of excellence by Aurora Sinai Medical Center. Laura has shown dedication to patients, colleagues, students, and community partners.
