Certification in care coordination and transition management is available through the Medical-Surgical Nursing Certification Board (MSNCB). Nurses who pass the exam earn the credential CCCTM® (Certified in Care Coordination and Transition Management).
The American Academy of Ambulatory Care Nursing (AAACN) offers exam study resources and contact hours, but does not administer the exam.
